head of the church, vatican is just a city where he lives. it is an island unto itself. but the terms are sometimes used interchangably.

If he meant that he did not respect the Church's teaching authority on matters of faith and morals in general, then he was not a very good Catholic.
With regard to the loafers, I have never heard of 'Prada', which I assume is a brand name, but as far as I know, the pope receives no salary so Sojourner may be right, they may have been a gift.
